Building families through donated embryos


This process involves several steps that must be completed before the embryos can be released to the recipient couple.

You must first schedule an appointment with your physician at Midwest Fertility Specialists’ Carmel office in order to complete the required FDA screening and testing.  This visit will include blood work for infectious disease testing, completing a questionnaire, and undergoing a physical examination.  All of the testing is at no cost to you, and we apologize for any inconvenience.

The second step involves reviewing and signing the informed consent.  Please review the consent, but do not sign it until we can witness your signature(s) at the visit noted above.  The signed consent is required before the embryos can be released to the recipient couple.

The final step prior to releasing the embryos is for you and your husband/partner to complete the history forms and bring them to your appointment at Midwest Fertility Specialists.  These records will provide a non-identifying personal history as well as a genetic/family history.  It is important that you NOT put your names on them.  These records will be assigned a number to keep you anonymous, and copies will be provided to the recipient couple for their records.

Midwest Fertility Specialists will also provide you with a consultation with our counselor if you would like to take advantage of this option during the donation process.  This is a service we provide at no cost to you, and we encourage everyone to avail themselves of this opportunity.
